# Calibrate Health CEO Rob MacNaughton

# Not the magic pill

A family member was dying of cancer, and Rob MacNaughton was trying to find someone to care for him at home. He was running a video game company at the time. He looked at home healthcare options in Portland, Oregon, and didn't like any of them.

"In retrospect, rather hubristically, I thought, you know what? I can do better," he told the CEO.com Podcast.

He quit, hired doctors and nurse practitioners to get him licensed, and hung out a shingle. The pitch was that his agency would answer the phone. That was enough. Within a few months, he had a few hundred employees, most of them hourly caregivers, and a few offices. He sold the business and spent the next stretch of his career on the other side of the table: investing in nursing homes, running hospital operations for a veterinary chain with 5,000 clinics, then working for a Blue Cross Blue Shield plan, where he says he learned how the money actually moves. He ran [two health tech companies](https://www.businesswire.com/news/home/20240208486867/en/CORRECTING-and-REPLACING-Calibrate-Names-Rob-MacNaughton-as-New-Chief-Executive-Officer?ref=ceo.com) after that, then went to Redesign Health to back other people's startups.

Calibrate hired him as chief executive in February 2024\. The company had [raised $100 million in 2021](https://www.forbes.com/sites/maggiemcgrath/2021/08/25/exclusive-telehealth-startup-calibrate-notches-a-100-million-series-b-from-founders-fund-and-tiger-global/?ref=ceo.com) selling weight loss programs to consumers. By the time MacNaughton arrived, it had [laid off staff, issued refunds over drug shortages, and been restructured](https://www.fiercehealthcare.com/health-tech/metabolic-health-startup-calibrate-taps-new-ceo-it-eyes-expansion-employer-payer-market?ref=ceo.com) under investor Madryn Asset Management, and founder Isabelle Kenyon had stepped down. The job was to sell the program to employers and insurers instead. MacNaughton spoke with CEO.com that fall, eight months in.

Calibrate's argument is that GLP-1 drugs work and that is the problem. "These medications are figuratively and literally not the magic pill," MacNaughton said. "If people are just on the drugs alone and then they come off the drugs, all the research shows that they'll gain the weight back and sometimes more."

So Calibrate wraps the prescription in a year-long program. A clinical intake. A wireless scale that reports every weigh-in. A coach every two weeks. A doctor at each dose change. A curriculum on food, sleep, exercise, and behavior. MacNaughton said members lost 16 percent of their body weight in year one and 18 percent by the end of year two, from a report covering [about 16,000 people](https://www.businesswire.com/news/home/20240131349389/en/Calibrate-Sets-New-Benchmark-in-Obesity-Treatment-Demonstrates-a-Remarkable-18-Sustained-Weight-Loss-for-Members-at-24-Months-Outperforming-GLP-1-Medication-Trials?ref=ceo.com). The year-two figure rests on 1,169 of them. He said Calibrate was the only company in the category publishing its numbers. The numbers are Calibrate's own, not peer-reviewed, and the most recent version, the [2026 Results Report](https://go.calibratehealth.com/hubfs/2026%20Results%20Report/2026%5FCalibrate%20Results%20Report.pdf?ref=ceo.com), reports 21 percent at year four. That figure comes from 620 members who stayed in the program four years and had complete data, out of more than 50,000 in the report.

"Obesity is like the centerpin in bowling," he said. "If you can knock it down, you can have an impact on a lot of other things."

The company is fully remote, a couple hundred people spread from Portland to the East Coast, and MacNaughton keeps Central Time hours to split the difference. He does not pretend the arrangement is free. Innovation in an office happens when you walk down the hall. Remotely, it needs a calendar invite, and the calendar is full.

"It almost sounds ironic to plan for spontaneity. But that's what we're trying to do."

On getting a strategy to travel from the CEO to the coach on the phone with a member: "When you think you've done it enough, you're probably halfway there."

On decisions: "Align doesn't necessarily mean agree. Align means that we as a team have decided to go this route, and the focus is then execution on that." Everyone gets heard first. Then the arguing stops.

On AI, he described a wall. Chatbots and call routing on the member side; nothing on the clinical side. Roadmaps no longer than 12 months because the technology moves too fast to plan past that.

In May 2025, Calibrate [replaced him](https://endpoints.news/weight-loss-startup-calibrate-replaces-ceo-in-leadership-shakeup/?ref=ceo.com). The chief financial officer and chief clinical officer left at the same time. Endpoints News called it the latest leadership reset at a once high-flying startup. Rob Rebak became the company's third chief executive since Kenyon stepped down. Nine days later, Skypoint, a healthcare AI company in Portland, [named MacNaughton](https://skypoint.ai/blog/skypoint-announces-key-executive-appointments-to-accelerate-growth/?ref=ceo.com) its president and chief operating officer.

Nine months after that, Calibrate announced it had [finished the move to enterprise customers](https://www.prnewswire.com/news-releases/calibrate-completes-transition-to-enterprise-focused-obesity-management-more-than-doubles-client-roster-302692378.html?ref=ceo.com), more than doubled its client list, and expected to be profitable for the first time in its history. Rebak gave the quote.

CEO.com ends every conversation by asking who gave the guest a chance. MacNaughton went back to his late 20s, just out of Harvard Business School, building CRM software for car dealers with a classmate named Thomas. Jim Lavalle, the CEO of a technology company, wrote them a check.

"It's not like we had pedigrees and track records back then, and yet he saw something in us, and I'll be forever grateful for that."

He writes those checks himself now, as an angel investor. He said he thinks about Lavalle a lot.
