# Collective Health CEO Ali Diab: Empathetic leadership and market disruption

# Ali Diab wanted a kinder insurance company

Ali Diab was hospitalized with a serious illness a little more than ten years ago. When he got out, half his hospital charges had been denied. He could not figure out why.

"For reasons that were very impossible, I would say, for me to understand at the time," he said on the CEO.com Podcast.

He had spent his career in technology. He ran product management and business operations at AdMob before Google bought it in 2010; before that, he held [roles at Goldman Sachs, Microsoft, and Yahoo](https://collectivehealth.com/company-overview/leadership/?ref=ceo.com). He wanted to start a kinder, gentler health insurance company. Then he decided he did not want to be an insurance company at all.

Collective Health, founded in 2013, administers health plans for employers that pay their own claims. American companies cover about half the country's healthcare bill, Diab said, and most of them have little say in what they are buying.

"If you're a big self-insured employer, I mean even if you're a massive self-insured employer, you don't really have a lot of control over how your network looks, what kind of programs are offered, especially if you work with a traditional health insurance company. We enable big employers, basically, to mix and match different kinds of networks, different kinds of solutions, whether it's a very targeted solution to take care of people with diabetes in your population, or fertility needs, or whatever it is. Then to very quickly evaluate what's working and what's not. Then negotiate, basically, with the people that are providing the service on the other side with a lot greater power, because you have the data to know what's costing what."

The company says it [serves more than 800,000 members](https://collectivehealth.com/company-overview/?ref=ceo.com) at employers including Red Bull, Uber, Zoom, and Live Nation. Health insurance has one of the lowest Net Promoter Scores of any industry, Diab said, and Collective Health's is an order of magnitude higher. When TIME published its first [ranking of the world's top health technology companies](https://time.com/7318020/worlds-top-healthtech-companies-2025/?ref=ceo.com) in 2025, Collective Health was [one of 58 out of 400 to earn an "Outstanding" designation](https://collectivehealth.com/announcements/collective-health-named-to-times-worlds-top-healthtech-companies-of-2025/?ref=ceo.com).

Asked why American health insurance is tied to a job at all, Diab went back to the 1940s.

"It relates to wage controls after World War II. Effectively, to keep wage inflation under check, the US government granted private enterprise the ability to provide health benefits and other benefits in a tax-protected manner. That's kind of just the prevailing way the system works. That being said, we do have a national health service. People think we don't have nationalized healthcare. We actually do. We have Medicare, and we have Medicaid."

For the 170 million or so working Americans in between, the private sector pays. Diab does not argue that it shouldn't. Private enterprise has served other industries well, he said. The difference is that the people footing the healthcare bill were never given the tools to buy intelligently, and he thinks employers can. Healthcare costs per person have grown at three to four times the consumer price index for about 25 years, taking the sector from eight or nine percent of GDP in 1980 to more than 20 percent today. "When I'm taking more money out of your wallet to pay for your insulin, or to pay for whatever drug that you have to take, then that's money that you don't have for other things."

Diab programmed neural networks early in his career. It has left him with a habit.

"I have a bit of an allergic reaction to the use of the term AI. I think it's a term that's highly misused and overused. What I would say generally is there's no question there is ample opportunity for the use of software and machine-learning-based techniques to improve everything from operational processes to customer experience. I think AI, in the most recent incarnation of it, in the sense of generative AI and large language models, definitely has its place. I don't think they're a solution for everything like a lot of people are saying."

On whether CEOs should be paying attention, he said anyone not thinking about it over the next five years is missing out, and anyone who hasn't been thinking about it for the last 20 already has. He counts himself an optimist. The loudest doomsayers, in his view, either have an agenda or don't understand the technology. He worries more about deepfakes than about a Terminator.

In February 2026, Collective Health [announced Collective AI, built with Google Cloud](https://collectivehealth.com/announcements/collective-health-collaborates-with-google-cloud-to-deliver-collective-ai-cai-reimagining-the-healthcare-experience-through-enterprise-scale-ai/?ref=ceo.com). It answers members' plan questions inside the company's app, assists customer service representatives on live calls, and guides employers through annual enrollment. The announcement says member health data is never used to train outside foundation models. In April, the company [hired Kaushik Bhaumik as chief revenue officer](https://collectivehealth.com/announcements/collective-health-names-kaushik-bhaumik-phd-as-chief-revenue-officer/?ref=ceo.com).

Asked about self-leadership, Diab described his mornings. He gets up early, makes his kids breakfast, and drives them to school when it is his turn. He works from the office a few days a week and from home the rest. He makes time for a trail run or the gym every day and calls it a religious practice. Evenings are dinner and homework with the kids. "Pretty boring. That's my life."

Family, he said, is the ballast for everything else. He keeps a three-to-four-hour block each day for reading, mostly trade publications and internal documents, and that reading is usually what tells him where to spend his time. The hardest part of being CEO of a company this size, he said, is that the list of things he could be doing is nearly infinite, and a lot of what he ends up doing is whatever is most immediate.

He has started companies before this one. They were not wild successes. He credits them with the humility that has let him do a better job at Collective Health, and he says his team reminds him daily of something not very smart he has done. On empathy, he pushed back on the caricature of the tyrant CEO. "You cannot motivate and orchestrate without having empathy. How can you relate to somebody when you're talking to them if you don't?"

Every conversation on the CEO.com Podcast ends with the same question, built on the premise that the chances one gives are just as important as the chances one takes. Diab named teachers in Menlo Park, professors at Stanford and Oxford, mentors at Goldman Sachs, Microsoft, and Yahoo. He named Jeff Weiner, his boss at Yahoo, who later ran LinkedIn and became an angel investor in Collective Health, and Jeff Immelt, the former GE chairman who [sits on his board](https://collectivehealth.com/company-overview/leadership/?ref=ceo.com). Both, he said, can tell him he is wrong without making him feel terrible about it.

He named his parents first. They were political refugees from Syria. His father was sentenced to death in 1963 for advocating a pluralistic and democratic form of government. They moved halfway across the world, finished their educations, and made a safe place for their two sons.

"I think about the sacrifices that my parents made," Diab said. "That's higher than anything else for me."
