Mochi Founder & CEO Dr. Myra Ahmad: How GLP-1s are transforming healthcare

On April 20, a federal judge in San Francisco ruled that Eli Lilly's lawsuit against Mochi Health can go forward. Judge Jacqueline Scott Corley had thrown out Lilly's first complaint in October for failing to establish that Lilly was harmed at all. The amended version cleared that bar, and two claims survived: unfair competition under California law and false advertising under the federal Lanham Act.
Lilly alleges Mochi Health, a corporation without a medical license, exercises control over the doctors who prescribe through it. It hires the physicians at the affiliated Mochi Medical entities, advertises for them, supplies diagnostic protocols, and trains their staff. It also, Lilly says, changed the dose and formulation of patients' compounded tirzepatide three separate times, in December 2024, March 2025, and September 2025, driven by shifting pharmacy partnerships rather than any clinical indication.
California prohibits lay corporations from practicing medicine, and Corley found the allegations plausible enough to proceed. The California Medical Association filed a brief against Mochi's motion to dismiss, according to Courthouse News.
The order is not uniformly bad for Mochi. Corley dismissed Lilly's civil conspiracy claim outright, finding no plausible agreement among Mochi Health, the Mochi Medical entities, and Aequita Pharmacy, all named defendants. Mere association, she wrote, is not a conspiracy. She also declined to infer that Mochi forced any physician to write a particular prescription, noting the allegations do not support that. Discovery is now open. Mochi said it was pleased with the ruling and "remain confident in our position."
Aequita is the other thread. A year before the ruling, Washington State ordered the Kirkland pharmacy to stop compounding after it failed to implement a state-approved plan to correct deficient practices. The commission alleged those practices included letting untrained and unqualified staff perform sterile compounding.
Mochi responded by publishing third-party certificates of analysis for recent batches, saying it would not resume work with Aequita until the state cleared it, and characterizing the state's concerns as matters of reporting structure and operations. The Washington notice describes something more than paperwork.
This is the company Dr. Myra Ahmad came to talk about on the CEO.com Podcast. She is a physician trained at the University of Washington who founded Mochi in 2022 and reached profitability within a year.
"We raised funding in March 2022," she said. "Then by late 2022, we became profitable and have continued operating off existing cash flow since then."
She raised once. Hims & Hers, Ro, and Noom took hundreds of millions of venture dollars into the same market. Mochi grew on its own revenue instead, and by July 2025 reported 500% year-over-year growth, licensure in all 50 states, and patients who had collectively lost more than 3 million pounds.
That November, it announced an expansion into 15 new specialty areas, putting a catalog of more than 120 conditions in front of over 500 affiliated providers. Ahmad describes the model as a marketplace rather than a pipeline. "I usually compare it to DoorDash versus Domino's." Patients choose the provider and the pharmacy; the company sells access, not a single product.
The lawsuit went unmentioned in the conversation. But her defense of compounding, the practice at the center of it, ran through the whole hour.
"Compounding is actually a longstanding and very normal part of pharmacy practice," she said. It already happens in IVF, pediatrics, and skincare, she pointed out, without anyone objecting. Applied to obesity drugs, it lets a provider adjust dose and formulation to a patient's side effects. "It's really personalized medicine."
The FDA has since narrowed the ground further, advising that a compounded drug combining a GLP-1 with another active ingredient such as a vitamin may still count as essentially a copy of the approved product. Ahmad says tailoring is the company's entire point.
On the medicine itself, her ground is firmer. "If someone comes to a doctor for depression or anxiety, providers discuss medications and treatment options openly," she said. "Historically, obesity wasn't treated that way." She has watched the category she works in become a culture-war object and does not hide her irritation. "Honestly, I've never seen a medication category receive this much vitriol."
The population data is on her side. Gallup finds the adult obesity rate has fallen from a 2022 peak of 39.9% to 36.4%, a decline it calls statistically meaningful and says tracks inversely with GLP-1 use. Current use has climbed from 3% of adults in 2024 to 11% this year. Ahmad credits the medications.
Only 19% of current GLP-1 users are on a compounded version, against 68% on brand-name products. And the traffic runs mostly one way for one reason: 35% of compounded users switched from a brand name, while just 10% went the other direction, and those moving toward compounded were twice as likely to cite cost.
Price is where the pressure lands, and it comes from an unexpected direction. Under the White House's November agreements with Lilly and Novo Nordisk, Medicare pays $245 a month for Ozempic, Wegovy, Mounjaro, and Zepbound against a $50 patient copay; cash buyers pay about $350 through the TrumpRx platform, and Medicare covers Wegovy and Zepbound for obesity for the first time. Cheap branded drugs erode the discount that made compounded versions attractive. Ahmad chooses to see the policy as an ally. "Anything that removes unnecessary middle layers from healthcare tends to improve access and reduce costs for patients."
Mochi has been adjusting all the same, adding generics to its catalog and, this month, replacing its flat medication rate with per-dose pricing that drops compounded semaglutide to $60 at the lowest dose.
Tariffs, the year's other macro story, mostly missed her. Compounding pharmacies manufacture domestically. "At one point, we had a quarter-million water bottles stuck on boats for months," she said. "Our biggest tariff story was cardboard boxes and shipping delays."
The company is fully in person in San Francisco, built its own medical records system, and leans hard on lawyers. "Healthcare is simply much more regulated than traditional SaaS businesses." Medical school showed her where the system fails patients. It taught her nothing about the money. "Doctors often graduate without understanding how money actually moves through healthcare systems."
Asked who gave her a chance, she named her team. "Every one of them had many other opportunities available to them, and they chose to help build this vision together."
